Q&A Hemorrhoidal diseases
What’s pile?
Generally, it categorized into 3 kinds: blind piles(hemorrhoid), bleeding piles(hemorrhoid) and anal fistula. Pile is a disease distressing a huge range of people. However ,the fact is, we have a hard time deciding to see a doctor as it is a disease developed in anus, the embarrassing part of our body.
What are the symptoms of pile?
The symptoms differ according to the type of pile but blind piles(hemorrhoid) has symptoms of bleeding, proptosis, and aching pain. bleeding piles(hemorrhoid) causes acute pain and bleeding during a bowel movement and it is led to stenosis of anus and anal fistula develops abscess when excreted issue and discharge are piled up. Piles can result in a major issue if we belittle them. Since bowel cancer and rectal cancer have the symptoms of the same sort, like bleeding, we need to be careful. There are also people who develop anemia due to continuous bleeding.
What kind of examinations are there to undergo?
Generally, we ask our patients to be in lateral position(lying on the side) and examine by using an anoscope and a proctoscope. We monitor the condition of the pile using jelly which contains anesthetics.
What’s hemorrhoid?
External hemorrhoid
External hemorrhoid means blind piles. There are 2 kinds of cushioning cuvenous plexus around the anus, called Internal hemorrhoidal plexus and external hemorrhoidal plexus, when they are enlarged and bulge due to stimuli caused by constipation etc., we call them hemorrhoid. Internal hemorrhoid is developed in internal hemorrhoidal plexus which is on the side of intestinum rectum, when it increases in size, it may develop a condition called impaction in which the internal hemorrhoid comes out and returns into the anus during a bowel movement. When it gets worse, it may develop a condition called impaction in which the internal hemorrhoid is not returned into the anus after coming out. Furthermore, external hemorrhoidal plexus is a pile developed in external hemorrhoidal plexus, it often forms blood clot and blood tumor accompanying acute pain.
What treatment approaches are there for internal piles?
There are 4 phases in internal hemorrhoidal plexus (called Goligher classification)
Phase I Hemorrhoid is only puffed up inside the anus
Phase II Hemorrhoid pops out of the anus during a bowel movement After defecation, it goes back into where they have been.
Phase III Hemorrhoid is popped of the anus and can’t be returned unless it is pressed into it with fingers and such.
Phase IV Hemorrhoid is popped of the anus all the time.
Hemorrhoid in phase I   Hemorrhoid in phase II

Hemorrhoid in phase III Phase IV
Treatments are divided into conservative therapy(no surgery, receives a follow-up examination) , Surgery and other therapeutic methods.
Please tell me about conservative therapy for hemorrhoid.
Conservative therapy: There are life therapy and medical therapy and provided for relatively mild piles.
Life therapy The five points to prevent piles
  1. sanitize the anus, soak in the bath and warm the bottom everyday.
  2. Avoid being in one position, for example, prolonged sitting etc.
  3. Cut back on Alcohol and food that are too hot or spicy.
  4. Avoid constipation and diarrhea, aim for a natural defecation habit.
  5. Be open to consult a doctor
Medical therapy We use suppository and unguentum that lessen pain, bleeding, and inflammation.
Also there are cases where we monitor the course of condition by prescribing oral medicine that improves the blood flow of hemorrhoid.
